You can tell them about your current situation and ask for an assessment for Continuing Health Care funding (for adults) or Continuing Care funding (for children). This will help the ICB to assess your current needs and see what support they are able to offer you. 

A: Yes, you should be able to choose which care provider supports you. Sometimes the Local authority or Intergrated Care Board will have to allocate your funding through a direct payment (LA) or personal health budget (ICB) for you to choose which care provider supports you. 

A: You may need to fund part, or all your care. Be sure to ask the ICB or Council if you need to make any payments towards your care
